Support Connection:
Assistance for Participants to implement their plan by strengthening the ability to connect with the broader systems of supports and understand the purpose of the funded supports and participate in the community. Abito Care and Support Services will assist a Participant to understand the aspects of the plan, assist with ongoing management of supports, and answer your questions as they arise.
Assistance with decision making, daily planning, budgeting, life planning, and goal planning:
Some of the supports we provide include the preparation of schedules -daily, weekly and/or longer term. This is to help make sure that the supports being provided are fulfilling each of a participant’s life stages transition goals. We can also help with the budgeting process so that expenses are planned for and paid for on time/by the due dates.
Coordination of Supports:
Further qualifications/experience required to strengthen a Participant’s ability to design and to build their supports, with an emphasis on linking the broader systems of supports across a complex service delivery environment. Coordination of Supports focuses on supporting Participants to oversee their lives, not just their services. This may include resolving points of crisis and developing resilience in the participant’s network.
Individual Life skill development:
The life skills options that are included in our improved wellness program are cooking, gardening and fitness activities. It also includes personal care (Self-Care) assistance, daily household activities, music and dancing, meditation, sensory, and holidays activities. These calming and engaging activities increase the sense of value and well-being.
